Diabetic Wounds
Diabetic wounds are a category of pathological wounds that occur as a complication of diabetes mellitus. Long-term elevated blood glucose levels can impair the body’s natural healing process, damage blood vessels, and lead to nerve damage. These complications make the skin more vulnerable to injury and reduce the body’s ability to detect and respond to wounds. Diabetic wounds are particularly common in the lower extremities, especially the feet, and they often develop into chronic ulcers that are slow to heal.

What are Diabetic Foot Ulcers (DFUs)?
Diabetic Foot Ulcers are open wounds or sores that typically develop on the bottom of the feet in individuals with diabetes. These ulcers are one of the most serious complications of diabetes and are often caused by a combination of peripheral neuropathy (nerve damage) and peripheral arterial disease (poor circulation).
How Does the Wound Happen?
In people with diabetes, nerve damage can lead to a loss of sensation in the feet. This means that cuts, blisters, or pressure injuries may go unnoticed. At the same time, poor circulation reduces the supply of oxygen and nutrients to the tissues, making it harder for the body to repair even minor damage. These small, unnoticed injuries can worsen over time, especially if pressure is repeatedly applied to the area during walking or standing. Without proper care, the damaged area can break down into an open ulcer.
What Happens If Not Treated?
- Infection development: The ulcer can become infected due to exposure to bacteria, especially in moist or contaminated environments. With poor blood circulation, the body’s ability to fight infection is compromised.
- Progression into deeper tissues: If ignored, the wound may extend beyond the skin into muscles, tendons, or bones, becoming more complex and harder to manage.
- Gangrene formation: In severe cases, poor circulation can lead to tissue death, known as gangrene. This can result in blackened, foul-smelling, or numb areas around the wound.
- Increased risk of amputation: If gangrene or uncontrollable infection sets in, partial or full amputation of the affected toe, foot, or leg may be required to protect the patient’s life.
- Sepsis (systemic infection): An untreated infection can spread through the bloodstream, leading to sepsis—a life-threatening condition that requires emergency care.
- Extended hospital stays and healthcare costs: Delayed treatment often leads to hospital admission, surgical interventions, and increased financial and emotional burden.
Neuropathic Ulcers
What are Neuropathic Ulcers?
Neuropathic ulcers are a specific type of diabetic foot ulcer that occur as a result of peripheral neuropathy. This form of nerve damage reduces or eliminates the ability to feel pain, heat, or trauma, particularly in the feet.
How Does the Wound Happen?
When a person with diabetic neuropathy walks or stands for extended periods, areas of pressure, typically under the heel, ball of the foot, or toes, can create skin stress. Because the person cannot feel pain, they may not adjust their position or notice developing blisters or skin breakdown. Over time, this repeated pressure leads to callus formation, which can crack and ulcerate beneath the surface, forming a deep and serious wound.
What Happens If Not Treated?
- Silent worsening of the wound: Due to the absence of pain, the person may not notice that the ulcer is getting larger or deeper until it becomes severely infected.
- Chronic infection: Because the ulcer goes unnoticed for a long time, bacteria can invade the wound and form chronic infections that are difficult to treat and may resist antibiotics.
- Structural foot damage: Continuous pressure on the damaged area can lead to deformities, such as Charcot foot—a serious condition where bones weaken and collapse due to nerve damage.
- Amputation of affected limb: As with other diabetic ulcers, untreated neuropathic wounds can lead to gangrene and require surgical removal of the affected foot or part of the leg.
- Loss of mobility: Advanced foot ulcers and structural deformities can significantly reduce the person’s ability to walk, work, or maintain independence.
Ischemic Ulcers in Diabetes
What are Ischemic Ulcers in Diabetes?
Ischemic ulcers in diabetes occur due to severe restriction of blood flow to the lower limbs, often caused by peripheral arterial disease. These ulcers commonly appear on the tips of the toes, heels, or other areas that are farthest from the heart, where the blood supply is weakest.
How Does the Wound Happen?
High blood sugar levels contribute to the narrowing and hardening of arteries over time. This reduces circulation and limits the delivery of oxygen and essential nutrients to the skin and underlying tissues. When minor trauma occurs, such as a cut, abrasion, or ill-fitting footwear, the skin in poorly perfused areas cannot heal effectively. Instead of regenerating, the skin breaks down, and an ulcer forms. Because of inadequate blood supply, these ulcers are often slow to heal or may not heal at all without medical intervention.
What Happens If Not Treated?
- Delayed or failed healing: Due to restricted blood supply, the wound is unable to receive enough oxygen or nutrients, preventing healing and increasing the risk of deterioration.
- Tissue death (necrosis): Without adequate circulation, surrounding tissues may begin to die. The skin may turn black, harden, or develop a foul odor—classic signs of necrosis.
- Rapid ulcer expansion: Even small injuries can rapidly worsen under ischemic conditions. The ulcer can grow wider and deeper in a short time, increasing the complexity of care.
- Increased risk of limb loss: If the blood flow cannot be restored through medical or surgical means, amputation of the affected toe, foot, or limb may become necessary.
- High susceptibility to systemic infection: The risk of infection spreading to other parts of the body is significantly higher, potentially leading to serious complications such as sepsis or multi-organ failure.
- Pain and reduced quality of life: Ischemic ulcers are often painful, especially when infection sets in, resulting in sleep disturbances, anxiety, and decreased quality of life.
