Malnutrition-Related Wounds
Malnutrition-related wounds are a type of pathological wound that occur when the body lacks the essential nutrients needed for tissue maintenance, repair, and healing. People suffering from severe or chronic malnutrition—especially protein-energy malnutrition—are at high risk of skin breakdown, poor wound healing, and spontaneous ulceration. These wounds are most commonly seen in elderly individuals, those with chronic illnesses, or people living in poverty or institutional settings.

What are Protein-Energy Malnutrition Ulcers?
Protein-energy malnutrition (PEM) ulcers are wounds that result from the body’s inability to maintain skin integrity and repair tissue due to a deficiency in both protein and energy (calories). These ulcers often develop spontaneously or from minor friction, especially in individuals who are underweight or suffering from chronic illness. The skin becomes thin, fragile, and prone to injury, and once an ulcer forms, it heals very slowly or not at all.
How Does the Wound Happen?
- When the body lacks adequate protein and calories, it cannot build or repair tissue properly. Skin and muscle begin to break down as the body uses its own tissues for energy.
- The skin loses elasticity, moisture, and strength, making it vulnerable to pressure, friction, or even spontaneous breakdown.
- In weakened individuals who are bed-bound or have limited mobility, even slight pressure or rubbing against a mattress, chair, or clothing can cause skin damage.
- The immune system is also weakened, increasing the likelihood of infections and inflammation that further damage the skin.
What Happens If Not Treated?
- Continued skin breakdown: Without nutritional support, the skin continues to deteriorate. Even small wounds may expand into large ulcers or multiple areas of ulceration.
- Delayed or absent wound healing: Due to the lack of proteins and nutrients necessary for tissue regeneration, wounds may remain open for long periods, becoming chronic and difficult to manage.
- Increased risk of infection: Malnourished individuals have a weakened immune response, making them more susceptible to bacterial infections. These can spread from the skin to deeper tissues or into the bloodstream.
- Tissue necrosis and complications: As the body fails to support healthy skin, tissue death may occur. Necrotic areas can lead to severe pain, foul odor, and require surgical debridement.
- Systemic consequences: Chronic wounds increase the body’s energy demand, worsening malnutrition in a vicious cycle. This can result in overall physical decline, immune failure, and, in severe cases, death.
- Loss of mobility and independence: Painful, untreated wounds can make it difficult to walk or move, especially in the elderly or bedridden, increasing dependency and risk of further health complications.
