Radiation-Induced Wounds
Radiation-induced wounds are a type of pathological wound caused by the harmful effects of radiation therapy, commonly used in cancer treatment. While radiotherapy targets cancer cells, it also damages healthy skin and underlying tissues in the treatment area. Over time, this damage can impair skin regeneration, blood supply, and immune response, leading to chronic, slow-healing wounds.
These wounds may appear weeks, months, or even years after radiation therapy, and they tend to be difficult to manage due to ongoing tissue breakdown and poor healing capacity.
Radiation Ulcers
What are Radiation Ulcers?
Radiation ulcers are deep, painful wounds that result from prolonged or intense radiation exposure to the skin. They typically occur in areas where the skin has received high-dose radiation, such as the chest, neck, pelvis, or breast. These ulcers are characterized by tissue loss and necrosis, and they can be resistant to conventional wound care due to persistent vascular and cellular damage.
How Does the Wound Happen?
Radiation disrupts the normal structure and function of skin cells and blood vessels. When healthy skin is exposed to radiation, it undergoes inflammation, followed by thinning, breakdown, and sometimes ulceration. The underlying tissues may also become fibrotic and poorly vascularized, making them more fragile. With repeated trauma or continued breakdown, an open ulcer forms. These ulcers are often painful, slow to heal, and prone to infection.
What Happens If Not Treated?
- Progressive tissue destruction: Without intervention, the ulcer may expand in size and depth, involving not only skin but also underlying fat, muscle, or even bone.
- Chronic pain and discomfort: Radiation ulcers are often associated with significant, ongoing pain that can affect daily activities and quality of life.
- Secondary infections: The open wound becomes a gateway for bacteria, increasing the risk of infection, which further delays healing and may lead to abscess formation or cellulitis.
- Delayed wound healing or non-healing state: Due to permanent damage to blood vessels and tissue structure, the ulcer may remain open for months or years without significant improvement.
- Necrosis or fistula formation: In severe cases, untreated ulcers can develop necrotic tissue or abnormal connections between organs and skin, particularly when radiation affects the pelvic or abdominal regions.
Chronic Radiation Dermatitis
What is Chronic Radiation Dermatitis?
Chronic radiation dermatitis is a long-term skin condition that occurs months to years after radiation therapy. It is marked by changes such as skin thickening (fibrosis), discoloration, dryness, and in severe cases, the development of ulcers or areas of skin breakdown. Unlike acute radiation dermatitis, which resolves after treatment, chronic forms can be permanent and progressively worsen over time.
How Does the Wound Happen?
Radiation damages the skin’s basal cells, fibroblasts, and blood vessels, leading to long-term changes in the skin’s structure and function. Repeated radiation exposure results in chronic inflammation and fibrosis, which limits elasticity and nutrient exchange. The skin becomes more fragile, and even minor injuries or friction can trigger breakdown and ulcer formation. The deeper tissues may also harden and lose their regenerative ability, increasing vulnerability to long-term skin wounds.
What Happens If Not Treated?
- Skin fibrosis and tightness: Continued inflammation leads to thick, tight skin, which can restrict movement and cause discomfort or pain, especially in joints or flexible areas.
- Wound breakdown and ulcer formation: Fragile skin is prone to tearing, and small wounds can quickly evolve into chronic, non-healing ulcers.
- Delayed wound repair: Fibrotic and avascular tissues do not respond well to normal healing stimuli, making even minor wounds persist for long periods.
- Tissue necrosis: Severely affected areas may develop necrosis—where the skin and tissue die—requiring surgical removal or specialized wound care.
- Increased risk of secondary skin cancers: Chronically irradiated skin may undergo cellular changes that raise the risk of developing radiation-induced skin cancers, such as squamous cell carcinoma.
