What is a Second-Degree Burn?
A second-degree burn is a more serious type of burn that affects both the outer layer of the skin (epidermis) and the layer beneath it (dermis). These burns are deeper than first-degree burns and are typically more painful and severe.
Common signs of a second-degree burn include:
- Redness and swelling
- Blisters that may ooze or break open
- Wet or shiny appearance at the wound site
- Intense pain or tenderness to the touch
Because the damage reaches deeper layers of the skin, healing takes longer, typically 2 to 3 weeks or more, and often requires medical attention to prevent complications.
How Does the Wound Happen?
Second-degree burns usually result from longer or more intense exposure to heat, chemicals, or other sources of skin damage. Common causes include:
- Scalding from hot liquids or steam: Spills of boiling water, soup, or oil can cause deep tissue damage quickly.
- Direct contact with flames or hot surfaces: Being in contact with fire, stoves, or heated appliances for even a few seconds can cause this level of burn.
- Severe sunburn: Extended sun exposure, especially without sunscreen, can lead to blistering sunburns that go beyond the surface layer of the skin.
- Chemical burns: Strong acids or alkalis used in cleaning or industrial settings can burn deeply into the skin.
- Electrical burns: Electric currents may burn the skin and deeper tissues, especially if the skin is wet or exposed for a prolonged time.
- Friction burns: High-impact rubbing against rough surfaces (like falling on a treadmill) can cause second-degree friction burns.
What Happens If Not Treated?
- Infection: The broken skin and open blisters create a risk of bacterial infection. Without proper cleaning and protection, the wound may become redder, swell further, or develop pus, which can lead to complications like cellulitis or even systemic infection in severe cases.
- Prolonged Healing: Second-degree burns take longer to heal. Without the right treatment (such as keeping the wound moist and protected), healing may be delayed, and the risk of deeper tissue damage increases.
- Scarring: These burns often lead to scarring if not managed correctly. Infection, excessive inflammation, or peeling blisters can make the skin heal unevenly, resulting in permanent marks or thick, raised scars (hypertrophic scars).
- Loss of Skin Function: Damaged skin may lose its ability to regulate temperature, retain moisture, or protect against external irritants in the affected area, especially if healing is disrupted.
- Hyperpigmentation or Discoloration: Improper wound care may result in uneven skin tone, dark patches, or light spots after healing, especially in sun-exposed areas or among individuals with darker skin.
- Severe Pain and Discomfort: Second-degree burns are painful. If left untreated, pain can worsen and become difficult to manage. Infected or exposed nerve endings in the dermis can increase sensitivity and discomfort.
